We are located on the third floor of building #970- Piedmont Physicians Center. Please arrive 10-15 minutes before the scheduled appointment to have plenty of time to fill out new patient paperwork.
New patient paperwork can be completed in advance through the patient portal. If you do not have an email address on file, the link may not work. In the office, paperwork will be available on an iPad; staff assistance is always available.
Plan on bringing:
- Insurance card(s)
- Driver’s license/ID
- List of current medications (including dosage and frequency)
- List of past surgeries
- If applicable, your most recent sleep study, hearing test or imaging reports as they relate to the reason for your visit
For any ear-related or balance issues, Dr. Athavale prefers that patients have a hearing test, which is included in the visit but will be performed in our audiology booth before seeing the doctor.
We also offer language translations through virtual calls with Proprio.
