Please arrive 10-15 minutes before your scheduled appointment to allow plenty of time to complete new patient paperwork.
Plan on bringing:
- Insurance card(s)
- Driver’s license/ID
- List of current medications (including dosage)
- List of past surgeries
- If applicable, the most recent sleep study, hearing test or imaging reports as they relate to the patient’s reason for visit.
New patient paperwork can be completed in advance through the patient portal. If you do not have an email address on file, the link may not work. In the office, new patient paperwork will be available on an iPad; staff assistance is always available. We also offer language translations through virtual calls with Proprio.
For any ear-related or balance issues, Dr. Athavale prefers that patients have a hearing test, which is included in the visit but will be done in our audiology booth before seeing the doctor.
